Fill out this FORM and a staff member will contact you with all of your questions!
ready to get started? -
Fill out Hands & Hearts for Horses' paperwork including the physician form. The physician form will need to be downloaded printed off and provided to your primary physician to sign. A staff member will get a hold of you to schedule a time for an evaluation once all paperwork has been turned in.
